Or is their birthday the day they were created? In this case, Sims of one family/household would all have the same birthday, which would kinda be unideal and not really fun to play I think.
Also, when playing with aging turned off, can your Sims at all celebrate their birthdays? Will they just have a party and eat cake and so on and not age up?

I think it’ll base it on the numbers of days left to age, so yes they’ll probably age the same day if created at the same time. It’s probably more ideal for sims born in game. If it’s the case it’ll be a way to tell if a sim is a twin/triplet or not.
