I tried to have it happen in my vanilla game and I couldn't get it to happen at all I took my sim to the Willow Creek gym for 12 in game hours during a blizzard and kept the camera outside with all walls up and just watched. Sims spawned in workout attire some shivered and even turned blue but immediately ran inside the gym. When they left they spun into cold weather outfits and walked to the despawn spot and disappeared. During a second blizzard I watched my neighborhood, my sim was in the Crick Cabana house in Willow Creek, during the blizzard almost all Sims walked by in cold weather gear. Wolfgang Munch walked by in his scout uniform apparently unaffected. The next day was calm but freezing, there wasn't even any blue in the thermometer it was so cold. Sims went by in their jogging clothes and many turned blue, but all of them managed to get to a spot where they despawned and disappeared without any harm.
I won't guarantee it won't happen in a vanilla game, but I couldn't replicate it in mine.
