Hi @kaym99 Have you thought of going back to an earlier save? This is done directly via your folders, if you can work with the date and time of when the sim was alive, this is the best idea, imo...
There's a good guide here to go back to an earlier save, at Carls Sims 4 Guide for directions on doing that and salvage from an earlier time before it happened
Hope this helps.

In addition to @Sashas0nic response have you looked in the relationship panel of other sims from that household to see if your ghost is there. If so just invite it over.

As @jpkarlsen said, look in the relationship panel to see if the ghost is still in the save. Just check the relationship panel of a sim that you know who knew the ghost. If the ghost is there, then you might be able to resurrect the sim and save them. Another way is send a chain letter on the computer if you have a sim with at least level 4 mischief skill since the choice panel will show you every sim in the save. You could also try hosting a party to see if the ghost shows up in the guest panel. If the ghost isn't there in any of those then you're pretty much out of luck as the sim has probably been culled. If you can't find them then you might want to do as @Sashas0nic said and revert to an older version of the save that still has the sim in it.
If you do see the ghost in another sim's relationship panel, the send chain letter panel or the party guest panel, then the issue is hunting them down. Ghosts who are not in a played household as a playable sim don't have phones, if I remember correctly, so you can't invite them over that way. I might be wrong on that though but you'll be able to see if it does work or not if you find them in one of the panels. Otherwise you will have to meet them in person.
A couple of suggestions to do that. I haven't ever done these things though so I have no idea if they will work or not but hopefully it will give you a place to start. I would go to the ghost's home to see if the ghost will show up. Adding the Spooky trait to the lot might help. Might have to stay a while and you might not get the ghost you are looking for.
You could also try going to a bar on ghost night and see if they show up. Again you might have to go more than once. And you can see if they will show up at a party too if they show up in the guest panel.
Hope this helps.

One way to get ghosts to come to you is to give your home lot the spooky lot challenge (used to be a lot trait until CotL). A ghost will show up every night. From what I can tell they sort of rotate as to which ghost shows up and the one you are looking for might show up one night. You just have to be patient. Then you can talk to them, ask them to join the household, resurrect them, etc. But it'll only work if the ghost is still in the game and hasn't been culled. In your case, if the ghost still shows in the exspouse's relationship panel, then he's still in the game. If not, then he's probably been culled.
Hope this helps.
@OnsenDreamscapes Hello! If you're on PC or Mac and don't mind using the MCCC mod, then here are the steps I take when I miss out on an urn (these are based on the most recent public version of MCCC). NOTE: Remember that mods are not supported by EA. Also, there are a lot of default settings when you add MCCC that will change your game, mostly for NPCs; the population module will begin moving NPCs into lots, having them get married and have babies, etc. So, if you're new to MCCC, it's a good idea to test the mod in a new save or use "save as" to preserve your current game until you can test all the features.
1- Play a sim who has a relationship with the ghost sim for whom you want the urn.
2- Go to the relationship panel, click on the ghost, choose the MCCC option -> Sim Commands -> Maintain Sim -> Revive Sim
3- Invite the newly revived sim to hangout, then when they show up use the friendly interaction "Ask to Join Household" (they should maintain all relationships)
4- Once the newly revived sim is back in the household and on the lot (the sim must be on the lot; best if they're inside the house), click on that sim, choose the MCCC option -> Sim Commands -> Maintain Sim -> Kill Sim (choose type of death from the list) This may seem a little harsh so you could let the sim live and let them die again naturally. However, this will give you the urn so you can move on with your game.
- @JonaO703 I used MCCC to kill a Sim because it was "his time" and he wasn't dying - but he literally just disappeared, no urn, no tombstone... hmmm. what to do to get that tombstone?
- @IanMcIntyre0264 Some weeks ago i used MCCC for the same. I marked a Sim to die.
Few minutes after the Sim leaved the Neighbourhoor, I got a Message about that Sim. Message said the Sim died. ( But he was not on Lot)
Few days later I found some Urns in a unplayed house in Moonwood mill. I am not sure if the Urns have something to do with the kill from MCCC - @666_Carbide_666 Ok, thanks. Maybe it was just a glitch or something wrong with MCCC, because last time I k!lled a Sim w/ MCCC it played the whole dying animation and gave me an urn. I'm like, okay then. But I keep all the tombstones cause I'm playing legacy and it's nice to see how far I've come, etc. 🙂 thanks!
- @IanMcIntyre0264 With MCCC there are more than one way to let a Sim die.
You can kill a Sim direktly, or mark him to die, so the sim will die later when he is not on lot.
