You may have tried this, but you didn't say. So, let's do a user folder reset by renaming or moving the "The Sims 4" folder elsewhere out of the game path. Start the game and a new one will be created. Check San Myshuno in the new save. If it is ok, then the game is loading as it should.
Next, copy or move your existing save to the new user folder. Start the game and load that save file. Check San Myshuno and if it is the same as you show in your pic, then it's the save that is bad. You might be able to save your households and some of your builds by saving them to your Library and adding them back in the new game. If it isn't and everything looks normal, then start loading in your mods and CC slowly. Test everything. If the problem comes back as you do that, you've found your problem file and should delete it. It might take another folder to get back to playing that save since the issue you are showing in the pick might not go away just by removing the bad mod or mods.
