As I have continued to troubleshoot, I have discovered a mod that is causing the issue. It is not the fault of the game and I have removed the mod. Consider this case closed.
The system is not working and I am not up to fighting with it. It is Sims 4 on the computer with all the latest patches as of 9/26/2022.
I do use mods and cc, but this issue just started after the last two patches. I have all packs installed. If a sim child is taking a bath and you save the game while they are doing so, the child is bugged as in every time you save after they get out of the bath, they will become nude. I tried repairing the game, disabling mods, and nothing would fix it. I tried leaving the lot, switching families, nothing worked. This happened twice. Both times, I ended up deleting the affected sim, and recreating them in the family.

Which mod was it, because I have the same probleme and don't know which mod it is
@StormForce1998 The mods broken in 2022 won't be the same as those broken now. Start at the current list (July 23, 2024): Broken/Updated Mods & CC . If you still need support after that, please make a new post.
