There are a number of reports for 6, but as far as I've seen, all of them are caused by MCCC:

I'm not seeing a thread for exactly what you describe in 2.

In the future, please look for existing threads for the issues you see and post in those.  For any issue where no thread exists (or you can't find one), create a new thread, but only one per topic.
