Sims4 doctors are faceles
Whenever i travel to the hospital my sims doctors are ALL faceless. Its just them. I do have a lot of mods but nothing stops this.

@sunysnbunnys What happens if you do a true vanilla test? Drag the Mods folder to the desktop, delete the file called localthumbcache, open the game to test, and exit without saving.
