When starting a relationship with NPCs (Married or not) I'd get an autonomous pop-up (sometimes with multiple sims) To get engaged. I absolutely loved this feature, whether intended or not. That mod is very broken. Remove it. It's probably causing you that issue and others. KS is gradually remaking the mod from scratch; see her website for the one part released so far.
