I have this issue as well. I have seen other people with it that say it is because of CC, but I do not have any CC in my game.
Both my character and Greg look like this. Greg looked normal to begin with, but then next time he spawned, his face was like this. My character looked normal during their first transformation, then next time they were broken.
I have tried repairing the game, playing without mods and loading the game from before this happened. It keeps happening anyway. One fix people have suggested is to go to CAS and unlink the forms and randomize the werewolf form, but that fix does nothing in my game.
It’s so annoying that they released a pack with a major bug in it

do you have sliders? that might be the issue.
What are sliders?

Someone on twitter had this problem this is what fixed it fore them for anyone wondering, it was the cc sliders i had loaded on to my sim! a simple fix is to unlink the forms and randomise the wolf form and it fixes it!
I saw, but I never had sliders, and unlinking and randomising the bugged werewolf doesn't do anything in my game.

I have never had any sliders installed in my game, nor any cc. Even with no mods what so ever and loading a save from before this issue first started appear, this issue still shows up. Not just on my sim, but on the other werewolves as well. Repairing, randomizing sim (with the forms unlinked)... Nothing fixes it. And I have even less idea on how to fix the townies. Right now I am even avoiding playing the game because I am worried it is going to be worse and worse the more I play with the pack, since more townies become affected.

Steps: How can we find the bug ourselves? You can easily spot the faces, it looks like they got squished when they are are in wolf form.
What happens when the bug occurs? It doesn't affect the game much but it concerns me how their faces looks like
What do you expect to see? whenever they transform into werewolves you can see their faces are distorted.
Have you installed any customization with the game, e.g. Custom Content or Mods? Yes
Did this issue appear after a specific patch or change you made to your system? No

I recently download a werewolf game pack and at first, the game was doing well but all of a sudden my sim and other NPCs are having the same issue as me, their faces are distorted and I repaired my game, and removed my cc, and made a new save but still doesn't work. Does anyone know how to fix this issue? Thank you.

I merged your post with the existing thread on this.
Try resetting your user files as described in the factory reset described in this post: http://answers.ea.com/t5/The-Sims-4/FAQ-Troubleshooting-Steps-amp-Known-Issues/m-p/3621199
You won't loose anything. Just make sure to not delete any files and make a backup.
After renaming, repair your game by right clicking on the game in Origin.
Try on a new game with nothing added back first (start new game). If it works, put back your save game and try again. If it still works, you can start putting stuff back.
- Options.ini (or just re-set all your settings manually)
- Tray Folder
- Screenshots/Videos/Custom music
- Rest of the save games (leave out slot_00000001.save which is the autosave and no longer needed - if you still have it)
- Custom content like clothing, hair, skins, make up ....
- ConfigOverride, if you have your own configuration files.
- Mods (one by one, making sure they all still work)
The rest does not need putting back, cause they are mostly cache files that recreate or log files that are not needed for the game.
