Whenever my Sims children's teeth fall, they always end up like this. Randomized clothes and hair and pitch black texture-less skin. I don't know how to fix it? I'm not sure if it's CC or Mods or...

Have you tested without mods? It could be either a game bug (I can't recall if this one is current) or a mod issue. No way to know if it's cause by mods if you don't remove them.
I'll give it a try- Hopefully it's relatively easy to solve, I have so many mods lol thank you
