Mermaid Faces and Body Glitch Issues

How often does the bug occur? Very often but unpredictable. Currently all the time with CC, more details below.
What is your current game version number? 1.70.84.1220
What expansions, game packs, and stuff packs do you have installed? Every Expansion and Game pack and Perfect Patio, Movie Hangout, Backyard, Vintage Glamour, Laundry Day, Moschino and Tiny Living
Steps: How can we find the bug ourselves? Any time they are a mermaid - through CAS, through cheats on MCCommandCentre as well as after eating Mermadic Kelp.
What happens when the bug occurs? The sim's face disappears on their mermaid form. You can see the hair and teeth unless you click remove teeth (if CC). After changing into Mermaid form, they also have weird outfits over the top of their tail that aren't there in CAS (see attachment photos)
What do you expect to see? Their face will disappear and only the hair and teeth (sometimes) will be visible. Often it could be fixed by unlinking the sims and giving them a face preset.
Have you installed any customization with the game, e.g. Custom Content or Mods? Yes, but while testing I removed all CC and the same thing happened.
Did this issue appear after a specific patch or change you made to your system? Neutral/Not Sure
I have seen other threads where some people haven't had their issues fixed after doing everything suggested. So far I have:
- Removed all CC - no difference.
- Only removed CC that has been known to cause this glitch - no difference.
- Factory reset without reinstalling - resulted in new mermaid Sims without CC working but when I opened old save files of mermaids (who used to have CC and don't now after taking all CC out) the same glitch and issues were still apparent.
- I have unturned a mermaid (eating two kelp and going in water) and returned them - the same issue occurred. This was done with and without CC.
- I am not overly familiar with S4S but attempted to find all broken/ unfixable mods and removed them from the game.
In the third attachment showing some sort of skirt around the mermaid - the skirt is not any CC I own. In the bath, the Sim appears to be wearing the skirt from the Island Living expansion pack.
I've not used the mermaid occult since January 2020 when I didn't have any issues at all with this sim form. I'd really appreciate any advice or help - I miss playing with normal-looking mermaids!!
