Yes, I always open the game when no other applications are open because I want it to run as fast as it can.
As for what goes slowly, generally the game lags a lot more, even in low graphics mode. These are under the same conditions when the game goes faster. I have a few mods and a little cc but like I said it works fine sometimes. Also, I suppose my Sims do take longer to start tasks (the bubble with the task doesn't descend into the active zone).

One of the times it's running slow, could you try to create a new save game from the Main menu and see if it makes a difference? If not, could you restart your computer and see if just after restarting it it runs better?
Could you tell us which macOs you have installed in the computer? Fully updated High Sierra?

Ok! I tested it. It seems as though it is the whole game running slowly, regardless of the save game I am using. I hope there is a way to remedy this!
