Is this happening in the virgin game without mods in the newly created game folder?
If yes check if your Windows is up to date

By the way I see you have OneDrive . You can protect game folders against moving them arbitrarily to OneDrive

Vanilla game- no mods. Windows is up to date as of this morning. I ran DxDiag as you suggested and no errors were found. Rebooted (shutdown and restart) and still the same.
Any other ideas? Thank you for the tips.
