cleanliness and sleeping/food issues. HELP ME!

How often does the bug occur? Every time (100%)
What expansions, game packs, and stuff packs do you have installed? cats and dogs.
Steps: How can we find the bug ourselves? I think it is only in my game,
What happens when the bug occurs? When I got it sims 4 game, it was working well for most of the time but then one day, one of my sims was very unclean, so I spent THREE HOLE DAYS cleaning her in the shower and then bar was up full, but the game showed that the bar was full but it was orangyred. It said and acted like she was SUPER dirty! That was my main issue, but another issue is all my sims have "going to sleep" in their queue and they don't actually do it. But when I tell them to cook, or eat leftovers, they get it out and put it back!!!! This is really annoying and i really hope you can find a way to fix this. Another thing with the sleep issue, i bought an instant cleanness drink (I bought 2) neither of them worked!!! Please find a way to fix this!!!
What do you expect to see? i expect my sim's cleanness bar does not stay always super dirty. i WISH my sim would not get out food and instantly put it back in even though he is super hungry.
Have you installed any customization with the game, e.g. Custom Content or Mods? Never used.
