This sounds exactly like my situation!
Looking through my DxDiag, I actually was having an issue with my graphics driver. I had to uninstall it and go online and reinstall it from the manufacturer. Your DxDiag doesn't have the same error that mine did, so I'm not sure if we have a similar issue or not? On mine it said "Display Tab 3: There is a problem with AMD Radeon(TM) Graphics device. For more information, search for 'graphics device driver error code 22'" I don't see that on yours because it doesn't show the display tab 2 and 3. May be worth looking into?
