@misscath90 I know someone else had that exact same issue but I can't remember what they did (or if they ever solved) nor can I find the thread atm.
Maybe try taking them to CAS and give them a new outfit or try the cheat cas.fulleditmode to edit the features.

Yeah, looks very similar to that other person. I really don't know if they ever resolved it
Did you also try aging the Sims up and then back down by using the cas.fulleditmode cheat ?
I tried that, it's even worse. And I even started a new game - the same thing when a child became a teenager.
Are you using custom content in your game ?
Just some mods like hair or piercing but nothing else. I mean I downloaded them.
Try removing them then to see if one of them is causing the issue
