If it was a normal sim I don't believe it would result in the picture above with the changed outfit. His skin is the Grim Reaper skin. He is a young adult and I don't want to mess around with his age. Though I heard the grim reaper unlike other sims games doesn't crash or break the game in the sims 4.
Yes I do use mods and one of them is script related but it is based on relationship decay.

Probably hard to reproduce but unless it happens on a vanilla game, I won't add it to the list
Not only script mods cause issues, all mods can cause issues.
I'm moving this to the general forum for now¨. I don't know if you can evict him and then delete and see if a new one is spawned.
Backup your saves before trying anything

I was able to evict him. Found another sim that moved in on their own. I'll test later if anything happens again with no mods.

There is actually an option to auto fill empty homes in the sims 4. The more I know. Still don't know why the Grim Reaper decided to move in.
