Please also post a screenshot of the in-game graphics options you're currently using.  I understand the settings are on high, but it's easier to have a visual reference and to double-check the other related settings.

@Applez  Some of the Sims 4 errors in your dxdiag could be related to the graphics driver, and given the obvious graphical issue here, it's a good idea to clean-uninstall and reinstall the driver.  Here's how:

If that doesn't help, please try running your secondary monitor, the one I'm guessing displays Sims 4 based on the in-game resolution you're using, at 120 Hz instead of 165.  The latter is technically an overclock, and while it should still work, I've seen this refresh rate not get along with Sims 4 before.

If that doesn't help either, please disconnect the other monitor, then reboot.  In this case, please test in both fullscreen and windowed modes.  I'm sure you don't want to play like this long-term, but it's a useful test.

Me too - it has been sporadic and restarting the game would solve it, but with the new patch it seems to be all the time, restarting makes no difference. I do have NVIDIA, but installed the latest version.

@simonevans1963  I don't see anything alarming in your dxdiag, although you do have a few generic Windows update errors.  It's best to clean those up with a couple of basic checks of your Windows system files:

• Hit Windows key-X
• Choose either “PowerShell (Administrator)” or “Windows Terminal (Administrator),” whichever option is offered
• Inside the window that appears, copy and paste “DISM.exe /Online /Cleanup-image /Restorehealth” without quotes, and enter
• The system will start validating soon. If it throws an error, please list it here
• After it reaches 100%, hit Windows key-X again
• Again, choose “PowerShell (Administrator)” or “Windows Terminal (Administrator)”
• Inside the window, copy and paste “sfc /scannow” without quotes, and enter
• Post the message you receive here

Restart your computer, hit Windows key-i, select Update & Security, and click the box to check for updates.  If any install, restart again afterwards.

For the issue itself, please set Windows to always use the Nvidia GPU with Sims 4.  Hit Windows key-i, select System > Display > Graphics settings, click Browse, choose TS4_x64.exe, and select the High-performance option.  Then restart your computer.

Please note that this is different from and more reliable than the Nvidia Control Panel setting, which Windows doesn't always respect.
