@kaninkvinnan Your dxdiag is full of serious crashes of one of the graphics drivers, which would explain the issues you're seeing. The driver for your Nvidia graphics card is very out of date, so try installing the newest one from Nvidia:

Run the installer as an admin: right-click the download and select "Run as administrator." Choose the Custom (not Express) install method, and check the box to perform a clean install. Restart afterwards and before trying to play.
However, it's possible that this is a hardware problem rather than driver-related. If the above doesn't help, try forcing Sims 4 to use the integrated Intel graphics chip. Performance would be significantly worse, at least relative to what the Nvidia card should be able to do, but it's a useful test. Open Windows Settings > System > Display > Graphics settings, click Browse, choose TS4_DX9_x64.exe from the list, and select the power-saving option. Then try to play and let me know whether you see the same on-screen glitchiness.
