You have to use the train station to get to the downtown are where the hospital is located.

I did take the train. I have to repay and rebuild the hospital.

There was a glitch that was allowing people to build the hospital before building the train station. The glitch was fixed with the latest update. You can contact support and see if they will give you back the money you spent so you can build the hospital again. Or if you saved to the cloud before you build the hospital you can try downloading that saved game.
