Have you tried to move the bath? Maybe place it in the middle of the room so there's nothing around the bath, and try again? Just for testing purposes, you can move it back to its place later.

Hi, I am having this problem too. I've tried what you suggested but it hasn't fixed anything. Its the same for all toddlers in all houses. They complete the action fine but the hygiene level doesn't change.
