Thank you for all your help.
It is a Sim I had made over, so I guess that stops the hair turning grey?
It was because she is an elder, everything else is right.

@chris19802010 Marriage is possible for two Sims of the same gender, so that is not the problem.
If the NPC is still young, you should find the Propose event amongst the romantic options.

The NPC SIM still has red hair - how else can we tell if thet are old?

@chris19802010 Senior NPCs have a hunched back.

Have you given the NPC a makeover?

Another question, is your playable Sim single or divorced? Sims can be engaged to multiple partners but only married to one Sim at a time.

    @chris19802010 You're welcome. You can try with another NPC. Just remember next time to complete the proposal event at level 4 and then make them marry ASAP.
    NPCs tend to age up very quickly, unfortunately.
