Missing access to a world with a prexisting game

I was wondering how I would get access back to a world I have some how lost access too? The world is listed in my worlds list and when I go to connect to that world, it gives me the choice to pick a faction, I already have a base on that world, and when I click GDI it wants me to pick a location and when I do, a pop up comes up saying "Name not available". Witch tells me that I already have a base on that world. My guess what happened is that I haven't played in a long time along with changing my Email address I use for EA messed something up somewhere and I have no clue how to get access back to my base on that world. I have screenshots of what I'm talking about if I need to share them.
