Bought Donuts using iTunes Gift Card...but received no Donuts?
Sometimes there is a delay. I had that problem. It fixed itself after a while. If it doesn't fix it, you will need to contact itunes.

So after emailing and setting up a live callback from iTunes support I've made a bit of headway. Received this email reciept for the purchase (edited out my personal info):

Order Total: $49.99
Billed To: Store Credit

The Simpsons™: Tapped Out, Store Full of 900 Donuts
Report a Problem ELECTRONIC ARTS In-App Purchase $49.99
Subtotal: $49.99
Tax: $0.00
Order Total: $49.99
Notice the tax is 0.00, related to, as Dromstul mentioned, FL not charging tax on iTunes downloads. Also received this letter from iTunes support further clarifying it:

Thank you for contacting iTunes Store Customer Support. My name is Melanie and it is my pleasure to assist you today. I hope all is well.
I see that your concern is about paying for the tax that should be included for your purchase of "The Simpsons™: Tapped Out". I can certainly appreciate your need to get this resolved. Let me help you out with this matter.
David, after checking, it appears that your purchase for "Store Full of 900 Donuts" was successful and luckily, it does not include any tax which means your credit $50 is enough to fund the purchase. You can confirm this by checking your Purchase history.

So the issue wasn't with the $50 not being enough for tax, the issue seems to just be a screw-up on behalf of either EA not adding the donuts (still not there as of this morning) or iTunes not forwarding the purchase info correctly or however it works.
I also set up a callback from iTunes support for about an hour from now, so I'll hopefully get it handled by then. I'll update when I have more info.
