After struggling with this problem for over a year, I finally found the fix. In my case, the low GPU usage was being caused by Windows’ full-screen optimizations.
Once I right-clicked on r5apex_dx12.exe (in the game folder), went to Properties → Compatibility tab, and checked "Disable full-screen optimizations", the game started using the GPU correctly again.
