Launching without Squad

On more than one occasion, with a 3 man squad marked ready, the game would launch without a team member. We've seen this in wild card and deathmatch. And to add insult to injury, if the team member quits the game, they're hit with a penalty, which wouldn't have occurred if we all launched together.

Not a good look.
