Can’t hear Footsteps around me or enemies until too late

Can’t hear own footsteps or players around me ; cannot hear gun **bleep** till they are too close ; I have tried different types of Audio settings ; I’m on console with headphones sounds horrible . What’s going on ? Elaborate
