Ok, I found out how to create the DxDiag report. Here it is:

There are a couple of things that you can try here:
- Lower the video settings slightly and see if there's any improvement
- Check your PC temperatures; overheating can usually cause stutters/freezes
- Clean install the graphics card driver.
- Disable any overlays that you may have active, such as Discord, Nvidia Shadowplay, and so on.
