I’m currently 3 Seasons into my Driver Career Mode & on Standard/High settings for Mechanical Faults, 50% Races & 16 Race Calendar. I had 1 DRS failure & 1 ERS failure in the first 2 races of Season 1 & yet to get a single minor or large failure 34 races later, there must be a bug not letting me get any failures!

Yep I have the same issue. No mechanical faults at all even with set to High. EA can you fix this bug.
