Coming up against the same squad in MM then DC'ed after kickoff

I am trying to play rivals / champs / any matchmaking and when I load into a game, I see the same team (pope in net, 2 silvers) despite coming up against different users. After kickoff, I am DC'ed and match is under review. 

Issue has persisted for about a week now.
