Every formation looks the same in career mode (PS4)

I've been playing a manager career for two seasons now, and all of a sudden my formation reverted to the original default of the team. What's worse is that in the team management section, no matter what formation I choose, the actual layout of the team does not change - it remains the default club formation. This happened overnight and I can't seem to find a solution. This is on PS4.
