PC player here. I have the same issue with this premium hair style after purchasing it from the store (all Clubs modes are affected). 

1. After loading it from the store I'm be able to use the hair model for one match. Immediately after finishing the first game, the hair model no longer loads (= bald).
2. This cosmetic bug appears both in the menus and in matches.
3. Changing the colour of the hair will also change skin colour of the pro above the neck.
4. So far, the only workaround I've been able to find is to restart FC 26 after each match and then reloading the hairstyle via store.
5. All 5 face make-up textures have a similar bug. In order for the make-up texture to load in-game, you need to enter the character customisation menu before entering a match. Unlike the premium hair bug, this issue is only present in-game (menus aren't affected). Also, the texture will still load for the active gaming session, but once the session is interrupted with a restart, the aforementioned process needs to be repeated.

I have the same issue. I bought an full pack for 1200 FC points and for what?? For nothing.  Fix this
