Unstoppable Force Evolution disappeared

The unstoppable force evolution has disappeared from all menus after I completed it one time.. The player used was a black Head-to-Head card. The remaining 4 repeatable slots are completely missing on both console and the Web App.

Upon further investigation, there are multiple people with this same issue.

When you update ultimate team please refresh my Ultimate Team profile server data to restore the missing repeatable Evolution slots ASAP. Thank you
