Monitoring Task Manager while playing shows that the game uses about 8-10GB of RAM when the game launches, and this steadily increases over 15-45 minutes to upwards of 25-30gb and eventually crashes ...

Sometimes get this one as well; DXGI_ERROR_DEVICE_REMOVED, as opposed to the aforementioned E_OUTOFMEMORY.
