Best guess is some sort of record scan and business logic scheduled process either didn't run, didn't pull anything back to process, or is erroring when it runs on a record from the scan. Maybe some deadlocking or recent change. I'd check that and run it on some example accounts in a testing environment (hopefully you can copy production data to a lower environment and debug it)
