When I started the most recent save file, I created a sim (as you do) and played with them for a little while before deciding to make them a vampire.
Once I made them a vampire I gave them a bit of a look change (Hairstyle, eye colour, outfit) and went along with it just fine.
Eventually they needed to shower, so I sent them to do so, and when they changed into the nude... their appearance was now back to how the sim originally looked when I first made them (before turning them into a vampire). And the same thing happens when they go to work, changes back to that initial look. And it seems to only change it back for those 2, if I select to "change outfit" the sim is back to how they should look.
No idea why this happens, I haven't used mods or cheats, since im on ps4 and would rather keep achievements active. And any "similar" problem I could find, their fixes didn't work.
