Dead sims are not greyed out in the relationship panel/Family tree

I am on the console PS5 to be specific had a sim die off lot collected the tombstone as that sims ghost to put it in a cemetery and sent her to the nether realm. I am doing a legacy run so I have a lot of dead sims. The one that died off lot and forced her to move on without doing the souls journey is the only one that is not greyed out.

I figured while I had a vanilla game going to test that hat bug I would test this again as well, this is with all mods removed (mod folder on desktop), cache cleared, last exceptions removed etc

Tested this in a new save with new sims, two sims drowned in the pond from get famous and showed properly as ghosts in the relationship panel (although not in the wants bubble or in the phone call screen)
I then had Nancy  Landgraab killed by a killer rabbit and she did not show as a ghost.
I traveled to Mt Komerebi and had Travis Scott and Marcus Flex freeze to death and they both appeared properly as ghosts
I then had Moira Fyres killed by a murphy bed and she did not appears as a ghost
I then had another sim freeze to death and they did not show up as a ghost in the relationship panel

All of these deaths were done through gameplay properly with no cheats except for Nancy whom I use the Kill -> slain by a rabbit cheat on. 

As you can see, it seems to be a bit random as to what does and does not properly trigger ghost visual vs no ghost. And sometimes they show as ghosts in some panels but not others such as pop ups vs relationship panel or the family tree.

I could keep going but I think this is hopefully enough to show that there is something amiss.
