As soon as my sims put their infant into back carriers, they take them out immediately if they have another action to perform.

I am having this issue too. The Sim can place the infant in the carrier but when asked to complete any action the Sim then tries to hand the infant Sim off to another Sim or put the infant Sim on the ground.
I have the same problem, I was so excited for the play mat and the back carrier and its so sad that it does not work properly.
The only time my sims keep the baby in the carrier is when I am cooking immediately after. I thought I could do all kinds of activities like gardening, laundry, talking while the baby is in there, but my sims instantly take them out every time.
