Not only active and played sims, everyone, including NPC Sims will also face Romantic satisfaction decays and eventually reach "very unsatisfied".
It's impossible to fix every couple in the game to prevent a sad mood for them. That's because some couples are unable to play (Ex. Scott family who own the bar in Henford)
It's another issue that makes me have to add the mods to fix it because no sign of fixing for almost a year.
