since when have you been trying ?
I didn't have any issues with getting twins at all but at times the randomizer can have it's own mind.
Have you tried a different family to see if they have the same issue ?

I have been trying for about 5-10 different families (different games)
I have also tried different combinations with one having the fertility trait, both of them only trying fertility treatments, both of them having the trait etc.
Could you define for a long time more in details ?
Would you be willing to share your save game ?
If so, please zip up this folder: documents\electronicarts\thesims4\saves and drop the file to this Mediafire-Link to upload.

The problem has been there for around 1 year, (after the bug with too many twins and triplets was solved)
I have tried on and off since then through the year, also with different combinations of the fertlit trait and treatments
That's of course up to you.
I don'r really know how to help you though. Doesn't look like a widespread issue (don't see many complaints either on the officials or here) and have no issues myself.
Since you've done all the usual troubleshooting, I'm kinda out of ideas.
Maybe someone else has an idea.
