I think the frame pacing issue is probably occurring within the game engine. I have tried various limiter values with my 4090, and the highest setting with smooth frame pacing is 100fps at 4K/ultra.

@afzSnickelfritz  the cable ya use is it a high speed hdmi cable ?
4K 3840 x 2160 30Hz Standaard with Ethernet 1.4
4K 3840 x 2160 60Hz High Speed 2.0b
4K 3840 x 2160 120Hz Ultra High Speed 2.1

If youre monitor/tv supports Display port , then buy a 1.4 display port cable and try again 😉
