After a complete reinstall I am still having issues. You can see several video and audio stutters in my first game of the day (worth watching for funny accidental KC placement and my number one 'randoms' hate - just standing out in the open):
These are actually quite mild stutters though, some are horrendous, lasting over half a second. I was also getting double audio and audio triggering a least a couple of seconds after the event (such as firing a KC).
I don't buy the 'failing HDD' theory, sorry. I don't have any problems with any other game, and I've only been having these issues (major rendering, invisible guns) since the last 'mega' update. When i start an Apex gaming session I have to go to the Firing Range to make sure my guns are rendering.
Sidenote - I'm getting a 'Wating for Pre-cache' message when logging in. Never seen that before and I've been playing since S4.

Mate, after all this time, they cant even get windows right, so id say its an ms issue
Have you updated the xbox lately?
Maybe try backward rolling the system update if you can?
