Yes, it is guaranteed to open an heirloom pack on your 500th Apex Pack if you haven't unpacked an heirloom yet.
Does the counter reset after unboxing?
Yes, the counter does reset after unboxing an heirloom you need to unbox again 500 Apex Packs for a guaranteed heirloom.
You can get lucky of course en get it before but the chance is less than 1%.
I haven't received my heirloom within 500th packs.
Most of the time the user has miscounted their amount of Apex Packs.
A handy way to count your apex packs is by using this Apex Pack counter.

Emm...I've had the same problem here
just bought 57500 coins to get 506 packs
and it’s nothing after opened it all......
- hayhor5 years agoHero@maximas1986 I have a theory that most people that feel they have opened enough fall into 2 categories-
Those that cannot count or count packs that do not actually count, such as event packs.
Those that only look at money spent and equate 100 coins to a pack. Chances are, many do not just open packs with all the coins. They probably buy some items. If you load 11500 coins, the max at once, you can open 115 packs, if you dont buy anything. But if you are like me, you may only load coins if you see an item or two you like. So you buy two items and wham 3600 coins gone, leaving only the possibility to open 79 packs.
Can some people have issues? Yes, sure they can but from what I've seen on here is usually one of the two above.- EA_David5 years ago

@hayhor seems to be on the right track.
I've personally looked into a lot of these, and the system does appear to be working correctly. If I ever do find evidence of it not working correctly that would of course be a major concern to be highlighted as a priority
There are of course some wild claims from folks who wildly overestimated pack purchases, but also a lot of cases where folks were diligent in tracking purchases and it's a lot less clear why the guarantee wasn't hit.
Key things are spending coins on event packs, direct purchases, and crucially, failed coin purchases. If, for example someone buys multiple packs of coins and one purchase gets rejected, they may reasonably calculate their coin purchases based on their intended purchase amount.
