Hitting Targets when Aiming Down Sight

Does your reticle actually have to be on the target when you are aiming down the sight to hit the target? I know that sounds strange, but I have noticed a lot of times when the player isn't really close to the target with the reticle and still hitting the target. FYI, I spectate a lot of games when I die to see how others are playing.

@Eques3n Spectating has lag. You will never see things exactly how they do. There is a cheat on PC called silent aim tho. Look it up and you'll understand why some don't even have to aim.
