In what situation will players be banned for grouping other teams maliciously?

@Rotating_Wingman Does this happen a lot to you? You could report them and they may get banned but surely this is rare. As with any teaming I'd save a video that clearly shows the teaming and report it through the website. I've done it for full 3 man teams teaming.
