I have been having controller disconnects multiple times a day. Right when a battle begins or there is a lot going on I will get a controller disconnect screen from apex even though my controller is fully on and responds to the Xbox Menu. The only way to fix it is to take the batteries out of my controller and restart the controller then apex recognizes the controller again which pretty much ruins your game if you are in a battle. Step up your game Apex.

This might be a silly question and I'm sure you've checked, but when you double-tap the home button on your Xbox controller to check the battery level is it fully charged? There are some steps below I recommend checking out before jumping into your next game to see if they clear it up. We can't have your controller go out in the middle of a firefight.
Xbox One:
Power Cycle the Console
Important Be sure to wait 10 seconds. This step resets the power supply.
Clear Mac Address
Go to your Xbox Settings and check to see if your controller has the latest Hardware Update.

Hopefully those steps above do the trick. I know it's not ideal, but reinstalling the game might be a good option if it continues to happen. The game files might have been corrupted during an update so it's worth a shot to see if it persists afterwards.

I have this same issue… my audio is also extremely choppy on my headset when playing.
fix your damn servers, I don’t enjoy playing with these issues.
Did you ever find a solution? My controller is disconnecting every 10 seconds on this game even tho I have new battery’s in, can’t even play the game
