I have the exact same issue. My solution was to drop my resolution and my settings to medium. I have a 4090 + 7950x. My resolution when the issue (TN spike) occurs is when I am at 4k (2160p) high/ultra settings. I drop my resolution to 2k (1440p) with medium settings and the issue is gone. I have done so many troubleshooting steps, like installing new components and completely reinstalling windows 10 & 11. Nothing helps but the quality drop for some reason. It is the dumb time nudge (TN) spiking and it's making the game unplayable.
