This actually happened to me once also when in conquest, although quite rare. Picking up another weapon from the ground and reswapping back usually fixes it. Not sure how to exactly reproduce the bug, though.

@NicDrywall Do you use M&K or controller on PC? Do you have any other device plugged in via USB as well?
