I just had it happen to me today. It was the first time I noticed it. I hit a sniper 2 times in the head from about 20m, in that new cluster mess map of TDM, and he headshot me half a second later. His scope should be bouncing all over; at least when taking shots to the noggin'.
It's probably happened to me before, but I second-guessed my shots or network, at greater distances.
