Keybinds change after being revived

Noticed this issue on smaller gamemodes (got it to happen on TDM, Squad TDM, Rush, and Domination) where, after going down and being revived, my weapon keybinds change. Most times, my primary and secondary will revert to 1 and 2 (I have cycle primary bound to 1 and no binding for equipping specifically primary or secondary) and my 2nd gadget, which is bound to 2, will change to mouse 4 (which is the same as my first gadget, causing a conflict). Screenshot attached of before and after as video is too large to upload. This happens reliably every time I am revived in these gamemodes. Doesn't seem to happen on conquest/escalation type maps.
